                                         Embalming   

    Except in certain special cases, embalming is not required by law. 
    Embalming may be necessary, however, if you select certain funeral
    arrangements, such as a funeral with viewing. If you do not want
    embalming, you usually have the right to choose an arrangement
    that does not require you to pay for it, such as direct cremation or 
    immediate burial.

Alternative Selections

 A. Forwarding of Remains to Another Funeral Home                        

             This charge includes local removal of remains, services of staff,
             embalming, and necessary authorizations. This charge does
             not include visitation, rites or ceremonies prior to forwarding of
             the body.

             The cost of additional transportation will be as noted under
             "Automotive Equipment" on your "Statement of Goods and
             Services Selected".

         B. Receiving of Remains from Another Funeral Home                 

             This charge includes temporary shelter of remains, basic
             service of staff and transportation of remains to cemetery or
             crematory. This charge does not include visitation, rites or
             ceremonies.

         C. Immediate Burials             

             1. Immediate burial in addition to the casket cost selected from
                our funeral home.
             2. Immediate burial included with a grey cloth covered softwood
                casket selected from our funeral home
             3. Immediate burial with casket provided by purchaser

            The charge for an immediate burial includes removal of remains,
             local  transportation  to  the  cemetery,  the  necessary author-
             izations  and  basic  services of staff.  This  charge does not
             include  the  use  of  facilities  and  staff  for  any visitation or
             ceremony, public or private, prior to burial. (SEE ITEM E BELOW)

         D. Direct Cremation            

             1. Direct Cremation in addition to the cost of a wooden casket
                selected from our funeral home
             2. Direct Cremation included with an Alternative Container
             3. Direct Cremation with an Alternative Container provided by
                 the purchaser.

             The charge for a direct cremation includes removal of remains, 
             local transportation to crematory (except weekends and
             holidays), necessary authorizations, and basic professional
             services of funeral director and staff. This charge does not
             include the use of facilities and staff for any visitation or
             ceremony, public or private, prior to the cremation. An
             additional $100.00 charge will apply for weekend and holiday
             cremations.

         E.  Memorial Services/Graveside Services/Spring Burials             

             If you wish to have a memorial service or graveside service in
             addition to direct cremation or immediate burial, the following
             charges may apply:
             1. Professional services in connection with arranging for
                 services, clergy, church, cemetery, etc.
             2. Professional services in arranging for graveside services,
                 clergy, cemetery, etc.
             3. Use of Funeral Home for memorial service ( 1 hr. )
             4. Attendant, coordination of spring burial with committal
                and/or military services
